Please review the following requirements for your appointment date

If you are having surgical procedure, don’t eat or drink anything past midnight the night before.
Due to COVID-19 no escorts or kids are allowed to enter the building at this time.
If you’ve had a positive COVID test in the last 10 days, please reschedule your appointment immediately.
A facemask must be properly worn at all times. Please see examples on how to properly wear a facemask.
